The service provides nursing care, support and advice to people living in their own homes or residential homes.
The service accepts referrals for patients aged 16 and above who require nursing assessment, care planning and care delivery in support of patients who are acutely, chronically or terminally ill including:
- Long term partnerships with and support of patients with chronic disease, and chronic degenerative disease, their families and carers
- Palliative and terminal care
- Wound management
- Promotion of self care through education and empowerment of individuals and their carers Promotion of continence and assessment of incontinence
- Health promotion, advice and support
Nursing assessments will include the needs of carers and/or family members.
Hours of operation
The District Nursing Service operates Monday to Sunday between 8:30am to 5pm 365/6 days a year.
N.B. The District Nursing Service does not provide an emergency service.

Specialist Health Visiting for Vulnerable Adults

The specialist health visitor focuses on vulnerable adults who have complex health needs in order that they may stay safe, be healthy, enjoy and achieve, make a positive contribution and prosper.
A vulnerable adult is a person aged over 18 years or over 'who is or may be in need of community care services by reason of mental or other disability, age or illness and who is or who may be unable to take care of themselves, or unable to protect themselves against significant harm or exploitation'.
The service operates Monday to Friday 9am to 5pm, except bank holidays.
